Appointments
Every attempt is made to schedule appointments according to the patient's needs. It is required to have a referral from a physician if you choose to use your insurance. At the time of the initial appointment, we at BioSport Physical Therapy will do our best to verify insurance coverage and obtain authorization for treatment. A parent or legal guardian must accompany anyone under the age of 18 at the time of their appointment.
What to Bring to an Appointment
- A written referral from a physician (MD) if it wasn't already faxed over to BioSport Physical Therapy by your referring physician.
- No referral is needed if the patient needs to pay for therapy out of pocket.
- Current insurance information and cards.
- Any insurance required referral number or claims number.
- The name of the adjuster if its work related.
- A list of medications you are currently taking.
- Comfortable clothing that allows easy access to the area being evaluated. We provide patient gowns as needed.
Insurance and Financial Procedures
As a convenience to our patients, we will file all primary and secondary insurance claims. Patient's insurance companies and our office can make mistakes, and it requires the assistance of three parties to make the process go smoothly. Please do not hesitate to ask for information regarding insurance coverage and payment plans.
We are providers for thousands of PPO/Medicare/Workman's Comp insurance companies. Please contact your insurance company, at the customer service number provided on your insurance card, with any questions regarding your physical therapy benefits.
Cash Paying Patient:
We accept cash paying patients, and a referral may not always be required.
Private Insurance:
Currently BioSport Physical Therapy is an in-network provider for several insurance carriers. We are actively working with other insurers to secure in-network status in additional programs.
Please contact our office to see if we are in your network and learn about our support for out of network patients. We are here to help you with your healthcare and exercise needs. Your well-being is our top priority!
As a courtesy to our patients, we will contact your insurance company to verify your benefits prior to making your first appointment. Should there be a discrepancy on benefit details reported, we encourage patients to also contact their carrier to be aware of their covered and non-covered rehab benefits.
